Kidney Transplant Care & Post-Transplant Nephrology
From pre-transplant evaluation through decades of post-transplant surveillance — Dr. Kottarathara provides longitudinal nephrology care for patients before and after kidney transplantation.
Pre-transplant evaluation
Patients reaching advanced CKD benefit from early transplant referral. The pre-transplant workup coordinates infectious disease screening, cardiac clearance, malignancy surveillance, and immunological assessment. Dr. Kottarathara works alongside regional transplant centers to streamline this process and minimize wait time on dialysis.
Post-transplant management
The first year after transplantation requires close immunosuppression titration — balancing rejection risk against infection, malignancy, and metabolic side effects. Long-term care extends into management of CNI nephrotoxicity, BK and CMV viremia, post-transplant diabetes, and cardiovascular risk reduction. Dr. Kottarathara's research on CLL-associated allograft AKI (NKF 2016) reflects ongoing engagement with transplant nephrology beyond standard practice.
Allograft surveillance
- Scheduled creatinine and proteinuria monitoring with rejection workup escalation pathways
- BK virus and CMV PCR surveillance with antiviral and immunosuppression adjustment
- Coordination with transplant centers on protocol biopsies and DSA monitoring
- Long-term cardiovascular and metabolic risk reduction
